What is Investigation of Reports

Use The Investigation of Reports module to monitor your operational environment for changes, and to manage safety and quality concerns that are reported. For each issue you manage, you can conduct risk assessments, classify issues, manage corrective actions for continuous improvement, conduct investigations, and more.

The reports that you manage come from all type of concerns, such as safety, security, quality, and so on. These reports indicate a specific "undesirable state" of operations. You will manage these reports in order to understand all mechanisms of the undesirable states so that you can prevent their future occurrence. In effect, you manage issues to weed out exposure.

In effect, this module allows you to practice a majority of your FAA Part 5 Safety Assurance requirement in once convenient place, and in one process.

The module fulfills most the FAA requirements for having a Safety Assurance Process. In this module you can:

  • Monitor the operational environment and SMS for changes;
  • Review data;
  • Analyze safety performance;
  • Assess safety performance;
  • Monitor the effectiveness of control measures; and
  • Practice continuous improvement.

Moreover, many of the functionality in this module is optional, meaning you can make each tab as sparse or featureful as you would like. 

Features of Indvestigation of Reports

The Investigation of Reports module has the following features:

  • Assess issue initially, reassessment, and closing risk assessment;
  • Communicate the report to organization;
  • Set security permissions for the report;
  • Set target closure date;
  • Perform root cause analysis;
  • Classify the issue with a number of classifications, such as Root Causes, Hazard, Human Factors, Type of Issue, etc.;
  • Link this report to other reports, sub-contractors, locations, etc.;
  • Integrate with SRM Process;
  • Create and manage corrective actions (Continuous Improvement);
  • Perform full investigations;
  • Process issue through validation process; and
  • Flag issue for safety committee review or audit review.

Hazard Risk Management Process

For the most part, managing reported issues in the Automation Tool consist of the following elements, performed, more or less, in the following order. Keep in mind that this is a general overview of the process, and there are many more tools and nuances involved in facilitating issue management:

  • A report is submitted from one of the many data acquisition modules, such as hazard reporting, email reporting, etc.;
  • Default manager(s) receive email notification of new report, with steps and appropriate links to manage the issue;
  • Managers assess the vulnerability of critical assets with risk matrix;
  • Managers classify the reported events;
  • Managers link associate the issue with previously reported issues, subcontractors, locations, etc.;
  • If a new hazard is identified, it can be created, thus prompting the FAA required SRM Process;
  • Subject matter experts identify and facilitate ways to reduce exposure with Corrective Actions (Continuous Improvement);
  • A more detailed investigation can be performed;
  • Lessons learned can be identified and communicated; and
  • The issue can be flagged for further validation and review.
